Please attach a copy of the registration certificate, copies of the championship diplom certificate

as well as the copy of the receipt to the entry form.
UNLESS THESE DOCUMENTS ARE PROVIDED THE DOG WILL NOT BE REGISTERED.
VACCINATION REQUIREMENTS
All dogs participating the show or other competitions must have a valid certificate of vaccination against
distemper and rabies. Vaccinations are valid one year. In case of the first time vaccination or expired
vaccination at least 30 days must have passed between the vaccination and the show or competition days.
